In order for Monmouth Cardiology Associates to obtain records from your healthcare provider a medical record release form must be completed. Below is a link to the medical records release form. Please print and complete the form. Once you have completed the form, you can fax, mail or hand deliver to the provider you are requesting records from. Once the provider receives the signed document they will forward your records to our office.
