Dear Patients,

It is with great difficulty and mixed emotions that I inform you that I will be moving to California at the end of September 2018.  My family and I are relocating to the East Bay of San Francisco, to be close to my wife’s family.  For over two years, I had the honor and privilege of taking care of you.  I know that we have accomplished a lot together, and I hope that you will continue to have a keen interest in maintaining your cardiovascular health.

I am committed to ensuring that you continue to receive the highest quality of cardiovascular care.  To this end, I would highly recommend that you continue your care at Monmouth Cardiology Associates, LLC.  The office will ensure that your follow-up remains continuous and that your future healthcare needs are met.  I would recommend follow-up with any one of the physicians at the practice.  Two physicians in particular have recently joined Monmouth Cardiology Associates, LLC and would be great candidates for your continued cardiovascular care.  Dr. Bharath Sathya is an advanced imaging cardiologist who trained at the National Institutes of Health.  Dr. Matthew Saybolt is an interventional cardiologist who trained at the University of Pennsylvania.  I have worked closely with both cardiologists, and they are excellent, compassionate and talented clinicians.

Although I would recommend that you continue with a physician of Monmouth Cardiology Associates, LLC, if you choose to seek your care elsewhere, a copy of your records can be transferred at your request or released directly to you.

I have greatly valued our relationship.  Thank you very much for entrusting your care to me.
